Most SharePoint problems are not SharePoint problems. They are a file structure lifted from an old server and a permission model nobody has looked at since.

How we structure it
Sites organised around how the organisation actually works rather than around the old folder tree. Permissions granted to groups rather than individuals, so access follows a role and changes when the role does. Metadata where it earns its keep, and not where it just creates work. Retention set deliberately per content type.
For clinical and care records the structural question is usually which documents should be in SharePoint at all — the clinical system is frequently the correct home, and copying records into a document library creates a second set of obligations for no benefit.
Sector note
Community-controlled health services and multi-site aged-care providers get the most from this, because document governance is examined directly at accreditation and because staff move between sites. Getting permissions right once removes a recurring class of risk.

Can you fix our existing SharePoint rather than rebuild it?
Often, yes. A permissions audit and a restructure of the worst libraries usually gets most of the benefit. We would rather do that than sell a migration you do not need.
